Care Available 24/7
Care is provided on an intermittent basis with visits made based on the plan of care developed by the CCH team in cooperation with the patient and family.
Short-Term In-Patient Care
In-patient care is available at a local acute care facility and/or skilled nursing facility based on the symptoms exhibited by the patient. Additionally, respite care is available at a skilled nursing facility for up to five days at a time based on the patient’s need.
Durable Medical Equipment, Supplies, and Medications
We provide quality medical equipment, medical supplies, and medications related to the terminal diagnosis.
Physical and Occupational Therapy Consultation
PT/OT contractors are available for consultation on an as-needed basis, and their services are approved by the CCH team.
